SCHEDULING

To schedule your appointment, please call any time during office hours. Our office staff will work with your schedule and give you the next convenient available appointment. For insurance purposes, it is important that you have a referral from your primary care physician before scheduling.

FIRST VISIT

Plan to arrive 10 to 15 minutes prior to your appointment time.

REFERRALS

Some insurance companies require a referral from your primary care physician prior to seeing an orthopedic doctor. We must have the required referral/authorization in place before we can schedule an appointment or surgery. Call your primary care physician if you have any questions about your referral.

INSURANCE

Because every insurance plan is different, information concerning co-pays and deductibles for each patient will be different. It is important that you understand your coverage and plan, so that you will know what expenses you will be responsible for. Your co-pay must be paid at the end of each visit. We accept all payment forms, including all major credit cards, ATM and debit cards, checks, money orders, and cash.

We file all claims under Medicare, Medicaid, and many HMO or PPO managed care plans. Once the payment is received from the insurance company, a statement will be sent to you with the remaining balance due. If you have private insurance (any insurance coverage or plan in which we do not participate), you will be responsible for filing your own claims. Our staff will provide the information necessary to send to your insurance company and will do everything they can to help.
